DA Form , Equipment Identification Card is the form that gathers information on the equipment used within the United States Army and is used for its identification. The latest version of the form - sometimes incorrectly referred to as the DD Form - was released by the Department of the Army DA in September An up-to-date fillable version of the Equipment Identification Card is available for download and online filing below or can be found through the Army Publishing Directorate website.
A DA accompanies each equipment record folder. It is used by dispatchers and operators of the equipment in order to service and upgrade it in a timely manner. All data on the Equipment Identification Card must be updated after each service. The form has to be replaced when it is no longer legible. Filing instructions are as follows: Block 1 requires a bumper number of the item of equipment. Block 2 is for the number of the model.
The noun or noun abbreviation is entered in Block 3. Block 4 requires the NSN of the equipment. Block 5 is for a serial number or registration number only for equipment managed by registration numbers.
This entry must be done in pencil only. Only AOAP equipment requires filling out this line. Block 7 is for specifying the period of time or mileage after which the equipment requires new servicing.
This information can also be found on the DD Block 8 is for stating after what period of time or mileage the equipment requires new lubrication.
The information is recorded on the DD Block 9 is for submitting the operator's last name and rank. This is a pencil entry. In case there is more than one operator, the block must be left blank. Block 10 is for submitting the last name and rank of the operator's leader or supervisor.
